Two options. NEC 406.4(D)(2) allows GFCI replacement with a "no equipment ground" label, which gives shock protection without a true ground. The better option is to re-pull the home run with a grounding conductor so the receptacles are properly grounded.
Almost always a mismatch between the dimmer and the LED driver. LEDs need a dimmer rated for low-wattage LED loads, and the lamp brand matters. We match the dimmer to the load and verify it across the dimming range.
Usually no. Standard 1/2 inch deep fixture boxes are not rated for the dynamic load of a ceiling fan. NEC 314 requires a fan-listed box. We replace the box during fan installation when the existing one is not rated.
